By submitting this form, I certify that I am applying for levelized billing at the address listed above with service provided by Farmers RECC. I understand and agree that my bill will be based on my average consumption for the most current 12 months usage. I further understand and agree that I may be removed from the levelized billing program for failure to pay all bills promptly when rendered. It is understood and agreed that my participation in this program will continue so long as I am a consumer or until such time as either party requests to discontinue this levelized billing agreement.
